Sending the booking form is a request. Your time is confirmed only when we reply by text, email or phone.

If you are running more than 15 minutes late, please call us on +1 (503) 555-0142; we may need to move you to the next free time.

Please let us know as early as you can if you cannot make it, so the time can go to someone else.

This website and its booking form are not for emergencies. For chest pain, trouble breathing, signs of a stroke or heavy bleeding, call 911.

The information on this site is general, not medical advice. Prices are self-pay starting points; what insurance covers depends on your plan. Walk-in waits and free times are estimates and can change quickly.
